Requesting an Appointment
To get an appointment, you need to complete our Patient Registration packet for Functional Medicine. If you have a secure email server, you can send the completed packet via email. If not, kindly mail or bring the packet to our office. Once received, we will contact you to book your appointment.
IMPT: The Registration Packet is due at least SEVEN (7) DAYS before your appointment. Late submission will result in appointment cancellation and forfeiture of the Scheduling Fee. A new Scheduling Fee will be required if re-booking.
An Initial Consultation is required prior to any form of therapy. It includes a review of past medical records, questionnaire, current medical problems, focused physical exam, review of diagnostic and treatment options, and a summary report. It normally lasts about 1.5-2 hours depending on the complexity of the patient’s medical condition. The Initial Consultation fee is billed in 15-minute increments. Please contact the office for an updated copy of our Financial Policy Agreement.
When scheduling an appointment, a NON-REFUNDABLE Scheduling Fee is required. Please read our Financial Policy Agreement for further information.
The Follow-Up Consultation is scheduled 6-8 weeks after the Initial Consultation. It includes a review of laboratory data, interval history, physical exam when indicated, review of diagnostic treatment options and a summary report. The visit normally runs about an hour, depending on the complexity of the patient’s problems.
Specialized lab tests may be ordered by the doctor. These may be covered by your insurance.
Metabolic Detoxification and Weight Loss Program
Dr. Comia offers a 10-day metabolic detoxification program that help remove toxins from your body so you can feel more energized and healthier overall. The detox program kick-starts with a 7-day vegetarian diet that includes protein powder enriched with sulfates. This protein powder helps bind toxins that are circulating in your system so they can be more easily eliminated.
Additionally, Dr. Comia’s detox program includes a supplement that helps eliminate waste from your body. This is part of the Clear Change® 10-Day metabolic detoxification program by Metagenics®. Clear Change is clinically tested to address metabolic detoxification and heavy metal metabolism.
Many women and men choose to complete the 10-day detoxification program followed by Dr. Comia’s medical weight-loss program, but you can also complete the detox program by itself.
Hyperbaric Oxygen Therapy
An Initial Consultation is required prior to therapy. It normally runs about ½ an hour but may extend.
The Fee Schedule for HBOT:
Single session = $110 per session
Package Plan = $500 for five (5) sessions
Fees are due at the time of appointment scheduling. Each session is a maximum of 1 hour.
All sessions once purchased are NON-REFUNDABLE and NON-TRANSFERABLE. Please read our Financial Policy Agreement.
Intravenous (IV) Therapy
An Initial Consultation is required prior to therapy.
Due to prescription requirements and increased cost of IV nutrients, FULL payment for the therapy is due at the time of appointment scheduling. The IV therapy fee is NON-REFUNDABLE and NON-TRANSFERABLE. The fee varies depending on the medical condition being treated. Please contact the office for an updated fee schedule.
XLR8 Cold Laser Therapy
A consultation is required prior to therapy.
The Initial therapy fee is $10 per minute. The duration varies and there is no guarantee that the desired results will be achieved.
The Follow-Up therapy fee is determined by the total contact time either with the doctor or with the medical staff. Please see our Financial Policy Agreement for further details.
Verju Body Contouring
An Initial Consultation is required prior to therapy to determine eligibility and the number of treatment sessions needed. It normally runs ½ an hour but may extend.
The following is the minimum number of successive treatments per body area that is required:
• For Body Contouring plus Cellulite treatment = a minimum of six (6) sessions
• For Cellulite treatment only = a minimum of three (3) sessions
The Verju laser therapy is NON-REFUNDABLE and NON-TRANSFERABLE.
Tempsure Therapy / Non-surgical Face Lift
An Initial Consultation is required to determine eligibility for the therapy and the number of treatment sessions. There is a fee for the consultation. It normally takes about half-an-hour.
The fee for the therapy is dependent on the area involved. Payment is due upon appointment scheduling. Please contact the office for updated rates.
Office Policies for All Consultation and Therapy Sessions
We do NOT participate with any insurance company. We do NOT file for insurance and do NOT provide procedure or diagnostic codes, no exceptions. You are responsible for payment in full. FULL payment is due at the time of service. We only accept cash (in U.S. currency) and credit card payments (Visa/MasterCard only).
Appointment cancellation/rescheduling may be done by phone. If you reach our voicemail, please leave your name, date of birth, and telephone number. You will be contacted to confirm the message. IF YOU DO NOT GET A CALL-BACK or AN EMAIL RESPONSE, IT MEANS WE DID NOT GET YOUR MESSAGE.
The only exceptions to the Cancellation/Rescheduling and the Missed Appointment policies are true medical emergencies, or a natural disaster. A physician’s note is required for documentation and consideration.
A true medical emergency is when an illness or injury places a person’s health or life in serious jeopardy and treatment cannot be delayed. Some examples of True Medical Emergencies:
• Chest pain accompanied by sweating, nausea, vomiting, shortness of breath, radiating pain that moves to the arm or neck, dizziness, or feeling that your heart is beating irregularly or too fast
• Severe bleeding that doesn’t stop after 15 minutes of direct pressure
• Broken or displaced bones
• Swallowing poison
• Suddenly not being able to walk, speak, or move a portion of your body
• Shortness of breath or difficulty in breathing
Phone and online consultations are welcomed. This is convenient for long-distance travelers. It is billed at our standard hourly rate, divided into 15-minute increments.
Email communication is viewed as billable time and is billed in 15-minute increments based on our standard hourly rate. Personal email use is the patient’s responsibility. A patient who emails protected health and personal information via email assumes responsibility for using their email.
There is a charge for photocopying of medical records; please call for updated rates. Narrative reports for lawyers, for disability or life insurance, will be billed at our standard hourly rate.
Delinquent Accounts: Any collection fees and attorney’s fees incurred because of a delinquent account will be the responsibility of that account. All balances over 30 days will be subject to monthly late charges (2% per month), and will be reported to the collection agencies.
