Integrative Medicine - New Patients


 

Requesting an Appointment

To request an appointment, please call (860)242-2200. Your phone call will be triaged to the Integrative Medicine practice or the Internal Medicine practice depending on the reason for the appointment.

 

Biomedical Intervention

This appointment is reserved for patients who have Autism Spectrum Disorder (ASD) and/or chronic illnesses (e.g. Heavy metal toxicity, IBS, mental health issues, autoimmune disease, cancer, Celiac/colitis, chronic infections, chronic pain syndrome, fibromyalgia, food allergy sensitivities, multiple chemical sensitivities, learning disabilities).

Initial Consultation

The first visit includes a review of past medical records, questionnaire, current medical problems, physical exam, review of diagnostic and treatment options and a summary report.  It normally lasts about 1.5-2 hours depending on the complexity of the patient's medical condition. The Initial Consultation fee is billed per 15-minute increments. Please contact the office for an updated copy of our Financial Policy Agreement.

Once received, we will contact you to book an appointment. We require a non-refundable Scheduling Fee of $125 to hold your appointment. This fee will be applied towards your total Consultation Fee.

Initial Consultation Packet

All NEW patients must submit the following forms at least 7 days prior to their appointment:

For Patients <18 yrs old

  1. ABTC Questionnaire
  2. Informed Consent
  3. Privacy Notice
  4. Release of Medical Records
  5. Vaccination Schedule - please get a copy from your pediatrician

For Adult Patients:

  1. Exposure History Form
  2. Informed Consent
  3. Privacy Notice
  4. Release of Medical Records

Follow-Up Consultation

The Follow-Up Visit is scheduled 6-8 weeks after the Initial Consultation. It includes a review of laboratory data, interval history, physical exam when indicated, review of diagnostic treatment options and a summary report. The follow-up visit fee is billed in 15-minute increments. The visit normally runs about an hour, depending on the complexity of the patient’s problems.

The following must be submitted at least 3 days prior to the scheduled appointment:

  1. Updated Rating Scale
  2. List of updated supplements/vitamins and medications
  3. Non-Refundable Scheduling Fee - applied to the Follow-Up Visit Fee
  4. Copies of new laboratory test results

Failure to submit these in a timely manner will result in cancellation of your appointment automatically and forfeiture of the scheduling fee.


Verju Laser Therapy

Please call the office so we can book you for a Physician Consultation. Kindly complete the forms below and submit prior to your scheduled appointment.

For NEW Patients, you will need to complete the following forms:

  3. Verju Financial Policy Agreement - available upon consultation
  4. Verju Medical Questionnaire
  5. Informed Consent
  6. Ad Authorization Form

For CURRENT Patients, you only need to complete forms 3, 4, 5, and 6 above.

Physician Consultation (15-minute visit)

The physician will meet with you regarding the Verju Laser Therapy, giving you the opportunity to ask questions or voice concerns. Dr. Comia will inform you of the number of treatment sessions you will need. Please call the office for the fee schedule.

The treatment sessions are scheduled on a weekly, bi-weekly, and tri-weekly basis. The fee for the whole Verju Laser treatment package is due by the first treatment session.


Hyperbaric Oxygen Therapy (HBOT)

Physician Consultation

An appointment with Dr. Comia is required prior to doing the HBOT sessions. The consultation normally runs about 15-30 minutes but may extend depending on the complexity of a patient's problem. The Consultation Fee is $125 per 15 minute; it is separate from the HBOT session fee.  The consultation appointment may be scheduled on the same day for the HBOT session, depending on availability.

  3. HBOT Financial Policy Agreement
  4. HBOT Office Policies Form
  5. Prescription for mHBOT from your current Primary Care Doctor

Fee Schedule for HBOT:

  • Individual Session ("Dive") = $110/session
  • Package Plan = $500 for 5 sessions (i.e. $100/session)

Intravenous (IV) Therapy

Physician Consultation

Prior to the Intravenous Therapy session, all NEW patients must consult with Dr. Comia. The consultation runs about 15-30 minutes but may extend depending on the complexity of a patient's problems. The Physician Consultation Fee is $125 per 15-minute increment.

Intravenous Therapy Sessions

The Fee Schedule for the IV Nutrient therapy varies depending on the medical condition being treated. The patient will be informed of the fee prior to initiating treatment. The IV Nutrient Therapy fee is separate from the Physician Consultation Fee.


General Practice Policies

Advance Biomedical Treatment Center does NOT participate with any insurance company. It is an out-of-pocket consultation. We do not file for insurance. We do not provide procedure or diagnostic codes. You are responsible for payment in full. We accept cash (in U.S currency), cashier's check, money order, VISA or MasterCard. Full payment is expected at the time of service. A $30 service charge will apply for checks returned from your bank.

Cancellation Policy/Rescheduling Policy

Initial Consultations:

  • We require seven (7) business days advanced notice when canceling/rescheduling an appointment.
  • If cancellation or rescheduling is done LESS than seven (7) business days prior to the scheduled appointment, we have the right to bill the hourly rate of $500.00 (Cancellation/Rescheduling Fee). The paid Scheduling Fee will be applied towards this Cancellation / Rescheduling fee and a NEW Scheduling Fee of $125.00 will be required when rebooking the appointment.

Follow-Up Consultations:

  • We require three (3) business days advanced notice when canceling/rescheduling an appointment.
  • If cancellation or rescheduling is done LESS than three (3) business days prior to the scheduled appointment, we have the right to bill the hourly rate of $500.00 (Cancellation/Rescheduling Fee). The paid Scheduling Fee will be applied towards this Cancellation / Rescheduling fee. A NEW Scheduling Fee of $125.00 will be required when rebooking the appointment.

Appointment cancellation and rescheduling may be done via telephone or email. If you reach our voicemail, please leave your name, telephone number, date and time you called. We will contact you to confirm that we received your message. IF YOU DO NOT GET A CALL-BACK, IT MEANS WE DID NOT GET YOUR MESSAGE.

Missed Appointment Policy

  • A fee $500 will be charged if a patient misses an appointment
  • The non-refundable Scheduling Fee will be applied towards this $500 charge above.

Special Consideration will be given to true medical emergencies. A written proof from a physician must be submitted for documentation and consideration.

A true medical emergency is when an illness or injury places a person’s health or life in serious jeopardy and treatment cannot be delayed. Examples include difficulty breathing, chest pain, a head injury or ingestion of a toxic substance. If a person is experiencing severe pain, that is also an indicator of an emergency.

Some examples of True Medical Emergencies:

  • Chest pain accompanied by sweating, nausea, vomiting, shortness of breath, radiating pain that moves to the arm or neck, dizziness, or feeling that your heart is beating irregularly or too fast
  • Choking
  • Severe bleeding that doesn't stop after 15 minutes of direct pressure
  • Fainting
  • Broken or displaced bones
  • Swallowing poison
  • Burns
  • Suddenly not being able to walk, speak, or move a portion of your body
  • Shortness of breath or difficulty in breathing

Delinquent Accounts

Any collection or attorney’s fees incurred because of delinquent accounts will be the responsibility of that account. All balances over 30 days will be subject to monthly late charges (2% per month).

Laboratory Tests

Lab fees are not included in our fees.  Payments for laboratory services are made directly to the lab.  Please direct payment questions to them. Advance Biomedical Treatment Center LLC is not affiliated with these laboratories and does not profit from the use of their services. Laboratory test kits are provided free by certain specialized labs. However, a shipping & handling fee may apply. Please call the laboratories directly.

Medical Record Copy Requests

There is a charge for photocopying of medical records; please contact the Center for updated rates. Narrative reports for lawyers, for disability or life insurance, will be billed at our standard hourly rate.

Phone and Electronic Consultations

Consultations may be conducted via phone or via Skype (ID: AdvBiomedTx) /Facetime. This is convenient for long-distance travelers. It is billed based on our standard hourly rate.

Email communication is viewed as billable time.  Any email that requires at least 15-minutes of the practitioner’s time will be billed incrementally based on our standard hourly rate.
